Кардиоренальный синдром (КРС) – патофизиологическое состояние, при котором острая или хроническая дисфункция одного органа ведет к острой или хронической дисфункции другого. Цель настоящего исследования – представить характеристику больных, у которых хроническая сердечная недостаточность ассоциирована с хронической болезнью почек. ПАЦИЕНТЫ И МЕТОДЫ. Обследовано 368 больных (187 – мужчин, 181 – женщина) с хронической сердечной недостаточностью (ХСН) II-IV ФК в возрасте от 18 до 84 лет (средний возраст – 60±10 лет). Большинство больных – 247 (67%) имели ХСН с сохранной фракцией выброса. Скорость клубочковой фильтрации (СКФ) рассчитывали по формуле MDRD, хроническую болезнь почек диагностировали согласно NKF K/DOQI, Guidelines, 2002. РЕЗУЛЬТАТЫ. СКФ составила 68,8±20,9 мл/мин/1,73 м2 (от 18,4 до 142,6 мл/мин/1,73 м2), при этом, у 136 (37%) больных СКФ была &lt;60 мл/мин/1,73 м2. Больные с ХБП были старше по возрасту, преобладали женщины, среди них было больше больных с сахарным диабетом (СД). 42 больных имели сахарный диабет и определить причинно-следственные взаимоотношения между кардиальной и ренальной дисфункцией было невозможно. ЗАКЛЮЧЕНИЕ. Нарушение функции почек наблюдается у одной трети больных с хронической сердечной недостаточностью (хронический кардиоренальный синдром). У больных с сахарным диабетом трактовать причинно-следственые взаимоотношения между патологией сердца и почек во многих случаях невозможно, эти клинические ситуации нецелесооразно расценивать как кардиоренальный или ренокардиальный синдромы. Необходима разработка единых критериев кардиоренальных взаимоотношений (кардиоренального и ренокардиального синдромов), в качестве возможного варианта авторами предложен свой вариант классификации.</p></abstract><trans-abstract xml:lang="en"><p>THE AIM. Cardiorenal syndrome (CRS) – pathophysiological state in which acute or chronic dysfunction of one organ leads to acute or chronic dysfunction of another. The purpose of this study - to present characteristics of patients who have chronic heart failure associated with chronic kidney disease. PATIENTS AND METHODS. The study involved 368 patients (187 – male, 181 – female) with chronic heart failure (CHF) II-IV FC at the age of 18 to 84 years (average age – 60 ± 10 years). The majority of patients – 247 (67%) had heart failure with preserved ejection fraction. Glomerular filtration rate was calculated by the MDRD formula, chronic kidney disease diagnosed according to NKF-K/DOQI, Guidelines, 2002. RESULTS. GFR was 68,8 ± 20,9 ml/min/1.73 m2 (from 18.4 to 142.6 ml/min/1, 73 m2), while in 136 (37%) patients had GFR &lt;60 ml/min/1.73 m2 . Patients with CKD were older, predominantly women, among them there were more patients with diabetes. 42 patients had diabetes mellitus and to determine the causal relationship between cardiac and renal dysfunction was not possible. CONCLUSION. Renal impairment occurs in one third of patients with chronic heart failure (chronic cardiorenal syndrome). In patients with diabetes mellitus revealed causal relationship between the pathology of heart and kidney in many cases impossible, these clinical situations are not reasonably regarded as cardiorenal or renocardial syndromes.
