Определить направленность изменений экспрессии белка aKlotho, фактора роста фибробластов 23 (FGF23), интактного паратиреоидного гормона (РТН) и параметров обмена неорганического фосфата (Pi) в экспериментальных моделях ранних стадий дисфункции почек. МАТЕРИАЛ И МЕТОДЫ. Для моделирования хронического повреждения почек выполняли резекцию 3/4 и 5/6 почек у крыс линии SHR, сроки эксперимента - 1 и 2 мес. Контрольные группы составили ложнооперированные животные. Определяли показатели мочевой экскреции неорганического фосфата (Pi). Экспрессию aKlotho в тубулярном эпителии оценивали иммуногистохимически. Концентрации FGF23 и РТН в сыворотке крови определяли методом иммуноферментного анализа. РЕЗУЛЬТАТЫ. Реализованные модели соответствовали клиническим стадиям 1-3 хронической болезни почек. Почечная экскреция Pi достоверно увеличивалась в группах животных, подвергнутых нефрэктомии. Концентрации РТН в контрольных и опытных группах достоверно не отличались, вне зависимости от сроков и характера воздействия. Не выявлено достоверных изменений концентрации FGF23 и РТН в сыворотке крови по мере нарастания степени повреждения почек и увеличения экскреции фосфата с мочой. Достоверное увеличение FGF23 выявлено только в экспериментальной модели 5/6 нефрэктомии. Уровень тканевой экспрессии aKlotho был достоверно ниже в экспериментальных моделях 3/4 и 5/6 нефрэктомий с различными сроками эксперимента по сравнению с контролем. Кроме того, существенное снижение aKlotho выявлено на самых ранних стадиях повреждения почек (группа ложнооперированных крыс SHR через 2 мес) в сравнении с контролем. ЗАКЛЮЧЕНИЕ. Изменения в системе FGF23/aKlotho существенно опережают развитие вторичного гиперпаратиреоза; на ранних стадиях хронического повреждения редукция aKlotho в почке является первичным событием по отношению к системному повышению FGF23; увеличение относительной и абсолютной экскреции фосфатов на ранних стадиях экспериментальной ХБП происходят независимо от aKlotho, FGF23 и РТН.</p></abstract><trans-abstract xml:lang="en"><p>THEAIM. To determine changes of aKlotho protein kidney expression, circulating levels of fibroblast growth factor 23 (FGF23) and intact parathyroid hormone (PTH) and the parameters of inorganic phosphate (Pi) exchange in experimental modeling of early stages of chronic kidney disease. MATERIAL AND METHODS. The experimental models the chronic kidney injury were 3/4 or 5/6 nephrectomy (NE) in SHR rats while sham-operated SHR rats served as control groups. The duration of experiments was 1 or 2 months. The indices of Pi urinary excretion were determined as well as renal aKlotho protein expression by immunohistochemistry, serum concentrations of FGF23 and PTH (by enzyme-linked immunosorbent assay). RESULTS. The implemented models corresponded to 1C-3C stages of chronic kidney disease. Renal excretion of Pi was significantly increased in the groups of nephrectomized animals. No significant differences were observed in the serum concentration of FGF23 and PTH between control and experimental groups. FGF23 levels were significantly higher only in model of 5/6NE in compare to control groups. In contrary, the renal expression of aKlotho protein was significantly lower in all experimental models of 3/4NE, 5/6 NE compared to the control (sham-operated SHR, 1 month). Moreover, a significant reduction of aKlotho protein was identified at the earliest stage of kidney damage among models applied that was sham-operated SHR, 2 months (vs. sham-operated SHR, 1 month). CONCLUSION.
