Изучение физического развития детей с идиопатическим нефротическим синдромом (ИНС) в основном включает оценку длины и массы тела на фоне глюкокортикоидной (ГК) терапии. Недостаточно изучены данные показатели до и после завершения лечения.</p></sec><sec><title>ЦЕЛЬ ИССЛЕДОВАНИЯ</title><p>ЦЕЛЬ ИССЛЕДОВАНИЯ: изучить особенности физического развития детей с ИНС в дебюте заболевания, на фоне проведения ГК-терапии и после ее окончания.</p></sec><sec><title>ПАЦИЕНТЫ И МЕТОДЫ</title><p>ПАЦИЕНТЫ И МЕТОДЫ. Проведен ретроспективный анализ физического развития 89 пациентов с ИНС, госпитализированных в Воронежскую областную детскую клиническую больницу №1 в 1998–2014 гг. методом расчета Z-оценок длины и массы тела в сравнении с региональными нормативами физического развития.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В дебюте ИНС значения длины тела 38,2 % детей и массы тела 41,2% пациентов были меньше значений здоровых детей, р&lt;0,001. У детей со стероидчувствительным ИНС (СЧНС) не выявлено отличий Z-оценок длины тела на фоне проведения ГК-терапии (0,17±1,06) и после ее окончания (0,28±1,22), р=0,794. Длина тела уменьшалась у пациентов со стероидзависимым вариантом (СЗНС) по сравнению с пациентами со СЧНС. Значения Z-оценок массы тела детей со СЧНС увеличивались при приеме максимальных доз ГК (0,94±1,59) и возвращались к предыдущим значениям после окончания преднизолонотерапии (-0,24±1,33), р=0,040. У пациентов с часто рецидивирующим течением СЧНС и СЗНС сохранялся избыток массы тела и ожирение после окончания лечения преднизолоном (р=0,009).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Физическое развитие детей с ИНС имеет отличия, связанные с ГК-терапией. Так как длина и масса тела детей с ИНС отличались от нормативных показателей до начала ГК-терапии, возможно влияние генетических факторов, данное предположение требует изучения. Результаты исследования могут использоваться для прогнозирования влияния ГК на физическое развитие у пациентов с ИНС и оценки прогноза заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION. Study of physical development (PD) of children with idiopathic nephrotic syndrome (INS) includes mainly assessment of body height and weight during corticosteroid (CS) therapy; specifics of these criteria before and after the treatment are not sufficiently studied. THE AIM: to study PD of children with INS debut during CS therapy and upon its completion.</p></sec><sec><title>PATIENTS AND METHODS</title><p>PATIENTS AND METHODS. A retrospective analysis of PD was performed in 89 patients with INS in Voronezh Regional Children's Clinical Hospital No.1 during 1998-2014 using method of Z-score of body height and weight in comparison with regional standards.</p></sec><sec><title>RESULTS</title><p>RESULTS. At the INS debut body height of 38.2% of the children and body weight of 41.2% accordingly were less than those of healthy children, p&lt;0.001. Children with steroid-sensitive nephrotic syndrome (SSNS) had no difference in body height Z-score during CS therapy (0.17±1.06) and upon its completion (0.28±1.22), p=0.794. Body height was less in steroiddependent patients (SDNS) compared to SSNS patients. Z-score body weight values in children with SSNS were higher during maximum doses of CS (0.94±1.59) and returned to previous values after the therapy (-0.24±1.33), р=0,040. Patients with a frequent relapsing SSNS and SDNS had overweight and obesity remained after prednisone treatment (p=0.009).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Physical development of children with INS has differences associated with CS therapy. Since body height and weight of children with INS were different from the norms before CS therapy, this could be influenced by genetic factors, which to be studied further.
