Злокачественные новообразования, в том числе почек, широко распространены в мире. Основным методом лечения опухолей почек является оперативное вмешательство, одним из его вариантов является нефрэктомия. Злокачественные опухоли различных локализаций могут приводить к разнообразным поражениям почек. Гломерулопатии различного генеза часто протекают бессимптомно, в связи с чем пациенты не обращается за медицинской помощью. Для диагностики гломерулопатий необходима оценка морфологических изменений с постановкой реакции иммунофлюоресценции и проведением электронной микроскопии. ЦЕЛЬ</p></sec><sec><title>Исследования</title><p>Исследования: провести анализ клинических проявлений, частоты и структуры гломерулопатий при новообразованиях почек.</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. В исследование были включены 141 пациент. Пациентам была выполнена нефрэктомия, определены суточная потеря белка, количество измененных эритроцитов в моче, уровень креатинина сыворотки крови, рассчитана скорость клубочковой фильтрации (рСКФ) по формуле CKD-EPI. Морфологическая оценка образцов включала в себя реакцию иммунофлюоресценции и электронную микроскопию. Для оценки статистической значимости взаимосвязи между переменными вычисляли непараметрический коэффициент ранговой корреляции Спирмена (Rs). Полученные результаты считали статистически значимыми при р&lt;0,05. РЕЗУЛЬТАТЫ. Гломерулопатии были выявлены у 41,8 % пациентов. В 10,6 % случаев наблюдались гломерулонефриты, и у 31,2 % пациентов была выявлена диабетическая нефропатия. До проведения нефрэктомии протеинурия была выше у пациентов с гломерулонефритами, после проведения оперативного лечения отличий уровня протеинурии не было. До проведения нефрэктомии не было значимых различий рСКФ, после операции показатель был выше у пациентов без гломерулярной патологии. Через год имелось значительное снижение рСКФ у пациентов с гломерулопатиями. ЗАКЛЮЧЕНИЕ. При исследовании операционного материала морфологу следует проводить оценку и изменений в ткани почки вне опухоли. Целесообразно совместное ведение пациента онкологом и нефрологом. Гломерулопатии были выявлены у 41,8 % пациентов. В 10,6 % случаев наблюдались гломерулонефриты, и у 31,2 % пациентов была выявлена диабетическая нефропатия. До проведения нефрэктомии протеинурия была выше у пациентов с гломерулонефритами, после проведения оперативного лечения отличий уровня протеинурии не было. До проведения нефрэктомии не было значимых различий рСКФ, после операции показатель был выше у пациентов без гломерулярной патологии. Через год имелось значительное снижение рСКФ у пациентов с гломерулопатиями.</p></sec><sec><title>Заключение</title><p>Заключение. Целесообразно совместное ведение пациента онкологом и нефрологом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Malignant neoplasms, including those of the kidneys, are widespread in the world. The main treatment for kidney tumors is surgery, one of its options is nephrectomy. Malignant tumors of various localizations can lead to a variety of kidney damage. Glomerulopathies of various origins are often asymptomatic, and therefore, patients do not seek medical help. For the diagnosis of glomerulopathies, it is necessary to assess morphological changes with the formulation of an immunofluorescence reaction and electron microscopy.</p></sec><sec><title>The aim</title><p>The aim: to analyze the clinical manifestations, frequency and structure of glomerulopathies in kidney neoplasms.</p></sec><sec><title>Patients and Methods</title><p>Patients and Methods. The study included 141 patients. The patients underwent nephrectomy, the daily protein loss, the number of altered erythrocytes in the urine, the level of creatinine in the blood serum were determined, and the glomerular filtration rate (CKD-EPI) was calculated. Morphological evaluation of samples included immunofluorescence reaction and electron microscopy. To assess the statistical significance of the relationship between variables, Spearman's nonparametric rank correlation coefficient (Rs) was calculated. The results obtained were considered significant at p&lt;0.05. RESULTS. Glomerulopathies were detected in 41.8 % of patients. Glomerulonephritis was observed in 10.6 % of cases and diabetic nephropathy was detected in 31.2 % of patients. Prior to nephrectomy, proteinuria was higher in patients with glomerulonephritis; after surgical treatment, there were no differences in the level of proteinuria. Before nephrectomy, there were no significant differences in glomerular filtration rate; after surgery, glomerular filtration rate was higher in patients without glomerular pathology. After one year, there was a significant decrease in glomerular filtration rate in patients with glomerulopathies. CONCLUSION.
