В условиях многопрофильного стационара на протяжение 28 мес в 2019–2022 годах проведено одномоментное скрининговое клиническое исследование, в которое включено 240 пациентов с ВИЧ-инфекцией. Из них у 160 человек была диагностирована ХСН. В зависимости от наличия ХБП со скоростью клубочковой фильтрации (СКФ) &lt;60 мм/мин/м2 больные с ХСН были разделены на 2 группы. 1-ю группу составили пациенты с ХБП (46 человек), 2-ю группу – без признаков ХБП (114 человек). Всем пациентам в рамках исследования проведено определение цистатина С сыворотки крови, N-терминального фрагмента мозгового натрийуретического пептида (NT-proBNP) плазмы крови, С-реактивного белка (СРБ) сыворотки крови, количественное определение белка в моче.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Распространенность ХБП среди больных с ХСН и ВИЧ-инфекцией составляет 58,82 %, что в 4 раза чаще, чем у больных с ВИЧ-инфекцией без ХСН. Факторами риска развития ХБП у больных с ХСН и ВИЧ-инфекцией являются курение, употребление алкоголя и наркотиков, наличие в анамнезе хронических и острых форм ИБС, фибрилляции предсердий, желудочковых нарушений ритма, сахарного диабета, хронического вирусного гепатита С, анемии и тромбоцитопении. Больные с ХСН и ХБП имеют более жесткую артериальную стенку – имеется значимое превышение значений индексов жесткости аорты и периферических артерий. Длительное употребление антиагрегантов и препаратов антиретровирусной терапии отягощает течение ХСН у ВИЧ-инфицированных проявлениями ХБП. Концентрацию NT-proBNP в плазме крови у больных с ХСН-равную или превышающую значение 683,65 пг/мл, можно рассматривать как диагностический критерий развития ХБП у ВИЧ-инфицированных с чувствительностью метода 75,0 % и специфичностью 75,4 % соответственно. Доказана зависимость СКФ от трех основных факторов, отражающих поражение сердечной мышцы – концентрации NT-proBNP плазмы крови, значений ФВЛЖ % и ИММЛЖ, которые вместе, будучи включенными в регрессионную модель, определяют 42,2 % дисперсии значений СКФ и являются определяющими в развитии ХБП у больных с ХСН на фоне ВИЧ-инфекции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>THE AIM OF THE STUDY</title><p>THE AIM OF THE STUDY: to identify risk factors associated with the course of chronic kidney disease (CKD), as the main manifestation of damage to the target organ – the kidney, in patients with chronic heart failure (CHF) infected with the human immunodeficiency virus (HIV).</p></sec><sec><title>PATIENTS AND METHODS</title><p>PATIENTS AND METHODS. In a multidisciplinary hospital for 28 months in 2019-2022, a one-stage screening clinical study was conducted, which included 240 patients with HIV infection. Of these, 160 people were diagnosed with CHF. Depending on the presence of CKD with a glomerular filtration rate (GFR) &lt;60 mm/min/m2, patients with CHF were divided into 2 groups. The first group consisted of patients with CKD (46 people), and the second group – without signs of CKD (114 people). All patients within the framework of the study underwent the determination of cystatin C in blood serum, the Nterminal fragment of the brain natriuretic peptide (NT-proBNP) in blood plasma, C-reactive protein (CRP) in blood serum, and the quantitative determination of protein in urine.</p></sec><sec><title>RESULTS</title><p>RESULTS. The prevalence of CKD among patients with CHF and HIV infection is 58.82 %, which is 4 times more common than in patients with HIV infection without CHF. Risk factors for the development of CKD in patients with CHF and HIV infection are smoking, alcohol and drug use, a history of chronic and acute forms of coronary artery disease, atrial fibrillation, ventricular arrhythmias, diabetes mellitus, chronic viral hepatitis C, anemia, and thrombocytopenia. Patients with CHF and CKD have a more rigid arterial wall – there is a significant excess of the stiffness indices of the aorta and peripheral arteries. Long-term use of antiplatelet agents and antiretroviral therapy aggravates the course of CHF in HIV-infected people with CKD manifestations. The concentration of NT-proBNP in the blood plasma of patients with CHF equal to or greater than 683.65 pg/ml can be considered as a diagnostic criterion for the development of CKD in HIV-infected people with a sensitivity of the method of 75.0 % and a specificity of 75.4 %, respectively. It has been proven that GFR depends on three main factors that reflect damage to the heart muscle – the concentration of NT-proBNP in blood plasma, the values of LVEF % and LVMI, which together, being included in the regression model, determine 42.2 % of the dispersion of GFR values and are decisive in the development of CKD in patients with CHF on the background of HIV infection.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>хроническая болезнь почек</kwd><kwd>хроническая сердечная недостаточность</kwd><kwd>вирус иммунодефицита человека</kwd></kwd-group><kwd-group xml:lang="en"><kwd>chronic kidney disease</kwd><kwd>chronic heart failure</kwd><kwd>human immunodeficiency virus</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Wyatt CM.
