Проанализировать гистологические результаты применения метода экстракорпоральной фотохимиотерапии при аллотрансплантации трупной почки. ПАЦИЕНТЫ И МЕТОДЫ. Экстракорпоральная фотохимиотерапия была применена для предупреждения острого отторжения почечного аллотрансплантата в раннем послеоперационном периоде у 20 пациентов. Контрольную группу составили 20 пациентов с парными основной группе трансплантатами без применения данного метода. Пациентам обеих групп проводили протокольную биопсию трансплантата на 30-е и 180-е сутки после трансплантации или при наличии показаний с последующим гистологическим заключением согласно классификации Banff 2007 года. Также приведен клинический случай использования экстракорпоральной фотохимиотерапии в качестве терапии гломерулонефрита трансплантата. РЕЗУЛЬТАТЫ. Установлено, что в основной группе по результатам биопсии ни у одного пациента не выявлено признаков отторжения трансплантата, в то же время у 4 пациентов группы сравнения гистологически подтверждено отторжение пересаженного органа различной степени по Banff 2007, два из которых в результате привели к потере трансплантата. Применение экстракорпоральной фотохимиотерапии в качестве терапии гломерулонефрита трансплантата показало, что метод может быть использован с целью нормализации функции трансплантата и способен остановить процесс развития фокального и сегментарного гломерулосклероза, что подтверждено данными гистологического исследования биоптата до и после проведения процедур. ЗАКЛЮЧЕНИЕ. Экстракорпоральная фотохимиотерапия обладает эффективностью и может быть использована в качестве адьювантной терапии для предупреждения отторжения почечного аллотрансплантата и в качестве терапии возвратного гломерулонефрита трансплантата.</p></abstract><trans-abstract xml:lang="en"><p>THE AIM: is to analyze the histological results of the method extracorporeal photochemotherapy in cadaveric kidney allotransplantation. PATIENTS AND METHODS. Extracorporeal photochemotherapy has been used as a prevention of acute renal allograft rejection in the early postoperative period in 20 patients. The control group consisted of 20 patients with paired to basic group transplants without using of this method. Patients in both groups carried out the transplant protocol biopsies at 30 and 180 days after transplantation or when indicated, followed by the conclusion of histological classification according to the Banff 2007. Also showed a clinical case of the use of extracorporeal photochemotherapy as a therapy of transplant glomerulonephritis. RESULTS. It was established that in the basic group biopsy did not show any evidence of the transplant rejection, at the same time in 4 patients in comparison group histology confirmed rejection of the transplanted organ varying degrees of Banff 2007, two of which have resulted ingraft loss. Applications of extracorporeal photochemotherapy as the treatment of transplant glomerulonephritis showed that the method can be used to normalize graft function and is able to stop the development of focal and segmental glomerulosclerosis, that was confirmed by histological examination of a biopsy sample before and after treatment. CONCLUSION.
