Нефрэктомия может быть выполнена открытым (ОН), лапароскопическим (ЛН) и робот-ассистированным (РН) способами. Все методы обеспечивают одинаково надежное излечение от рака, однако в последние годы отмечена тенденция к более широкому применению малоинвазивных методик. Нами было предпринято исследование с целью сравнить все три способа выполнения нефрэктомии у больных с локализованным раком почки. ПАЦИЕНТЫ И МЕТОДЫ. Пациентам с раком почки в стадии T1-2N0M0 было выполнено 73 открытых, 10 лапароскопических и 23 робот-ассистированных нефрэктомий. Оценивали основные показатели, в том числе концентрацию креатинина сыворотки крови и скорость клубочковой фильтрации, длительность операции, объем кровопотери, длительность послеоперационного койко-дня. РЕЗУЛЬТАТЫ. Средняя продолжительность ОН была ниже, чем ЛН и РН (139,5; 184,5 и 152,5 мин соответственно). Средний объем кровопотери при ОН (447 мл) был достоверно больше, чем при ЛН и РН (302 и 213 мл соответственно). Прирост сывороточного креатинина составил 44,7, 52,2 и 52,9% для ОН, ЛН и РН, соответственно. Послеоперационный койко-день был больше при ОН (12 дней), чем при ЛН и РН (8 и 7 дней соответственно). ЗАКЛЮЧЕНИЕ. После ОН, ЛН и РН возникает начальная стадия острого повреждения почек. Несмотря на меньшую продолжительность вмешательства, в вопросах кровопотери и восстановления пациентов после операции, ОН уступает ЛН и РН. Предпочтение следует отдавать малоинвазивным методикам, которые обеспечивают высокие и сопоставимые между собой результаты.</p></abstract><trans-abstract xml:lang="en"><p>INTRODUCTION. Nephrectomy can be performed by open (ON), laparoscopic (LN) or robot-assisted (RN) method. All methods provide identically certain cancer treatment, however, in recent years the tendency for wider application of minimally invasive methods was noticed. AIM OF RESEARH: to compare three methods of nephrectomy in patients with localized kidney cancer. PATIENTS AND METHODS. We performed 73 open, 10 laparoscopic and 23 robot-assisted nephrectomies to patients with kidney cancer stage T1-2N0M0. We evaluated main factors such as serum creatinine concentration and glomerular filtration rate, surgery duration, extent of blood loss, post operation bed day period. RESULTS. Average duration of ON was lower than LN and RN (139,5; 184,5 and 152,5 min respectively). Average extent of blood loss at ON (447 ml) was significantly higher than at LN and RN (302 and 213 ml respectively). Increase of serum creatinine was 44,7%, 52,2% and 52,9% for ON, LN and RN respectively. Post operation bed day was longer after ON (12 days) than after LN and RN (8 and 7 days respectively). CONCLUSION. After ON, LN and RN appears early acute kidney injury. In blood loss questions and patients post operation recovery ON conceded LN and RN in spite of less operation duration.
