Изучить структурно-функциональные параметры сердечно-сосудистой системы в зависимости от уровня суточной альбуминурии у женщин с метаболическим синдромом (МС) в перименопаузе. ПАЦИЕНТЫ И МЕТОДЫ. В исследование было включено 67 женщин в перименопаузе с МС. Рассматривались две группы: основная - пациентки с МС и МАУ (n=34) и группа сравнения - с МС и нормоальбуминурией (НАУ) (n=33). Всем больным кроме определения суточной экскреции альбумина с мочой и расчета индекса массы тела (ИМТ) были проведены следующие исследования: суточное мониторирование артериального давления, определение толщины интима-медиа общей сонной артерии (ТИМОСА), проба с реактивной гиперемией, эхокардиография. РЕЗУЛЬТАТЫ. Ремоделирование миокарда у женщин с менопаузальным МС и МАУ наблюдалось в 88,2% случаев, преимущественно по типу концентрической гипертрофии (50% случаев). Средние значения массы миокарда левого желудочка (ММЛЖср.) и индекса массы миокарда (ИММср.) были выше в основной группе по сравнению с группой с МС и НАУ: 242,10±18,70 и 162,80±7,50 г (р&lt;0,01); 121,60±7,70 и 95,20±3,40 г/м2 (р&lt;0,01) соответственно. Нарушение эндотелийзависимой вазодилатации у женщин с МС и МАУ выявлялось в 76,5% случаев. У пациенток основной группы ТИМОСА была достоверно больше по сравнению с женщинами с МС и НАУ: 0,89±0,05 и 0,72±0,05 мм (р&lt;0,05) соответственно. Влияние систолического и диастолического артериального давления на МАУ подтверждается данными линейного регрессионного анализа. ЗАКЛЮЧЕНИЕ. МАУ при МС в перименопаузе ассоциирована с концентрической ГЛЖ, дисфункцией эндотелия и увеличенными значениями ТИМОСА.</p></abstract><trans-abstract xml:lang="en"><p>THE AIM. To study structural and functional features of cardio-vascular system depending on the level of daily albuminuria in women with metabolic syndrome (MS) at perimenopause. PATIENTS AND METHODS. A total of 67 women in perimenopause with MS were selected and divided into two groups: general - patients with MS and MA (n=34) and control group - with MS and normoalbuminuria (NA) (n=33). All patients were performed evaluation of daily albumin excretion by urinary way and calculation of body mass index (BMI). Also they were performed following examinations: arterial pressure daily monitoring, common carotid artery intima-media thickness test (CCAIMT), test with reactive hyperemia, echocardiography. RESULTS. Cardiac remodeling was observed in menopausal women with MS and MA in 88,2% cases, mainly by concentric hypertrophy type (50% cases). Mean left ventricular mass and left ventricular mass index were significantly higher in general group as compared to MS and NA group (242,10±18,70g and 162,80±7,50g; (р&lt;0,01). 121,58±7,70g/m2vs 95,16±3,40g/m2; P=0,002 respectively). Endothelial dependent vasodilation disorder inwomen with MS and MA was revealed in 76,5% cases. In patients of general group CCAIMT was accurately higher than in women with MS and MA: 0,89±0,05mm and 0,72±0,05mm (р&lt;0,05) respectively. The effect of systolic and diastolic arterial pressure on MA is verified by linear regression analyses data. CONCLUSION.
