Сравнительное описание почечных повреждений, выявленных у пациентов с АГ и умеренной протеинурией, у которых при изучении биоптатов почек был выявлен гипертонический нефроангиосклероз или же мезангиально-пролиферативный гломерулонефрит. ПАЦИЕНТЫ И МЕТОДЫ. В разработку включен 81 пациент, у которых показанием к выполнению нефробиопсии служило наличие артериальной гипертензии (АГ) в сочетании с умеренной протеинурией. Были сформированы две группы сравнения– больные с эссенциальной гипертензией (ЭГ, n=42), у которых по морфологическим данным не было сомнений в наличии нефроангиосклероза, и пациенты с мезангиально-пролиферативным гломерулонефритом (МзПГН, n=39). Нефробиоптаты исследовались на уровне стандартной световой микроскопии. Дополнительно проводилась полуколичественная оценка ряда признаков повреждений почек. РЕЗУЛЬТАТЫ. Пациенты с ЭГ отличались большим возрастом, длительностью АГ и величинами артериального давления (АД). При ЭГ уровень скорости клубочковой фильтрации (СКФ) был значимо ниже, чем у пациентов с МзПГН при сравнимых значениях протеинурии. Тяжесть неспецифических гломерулярных, сосудистых и тубулоинтерстицальных повреждений почек у больных с ЭГ также была выше, чем у пациентов с МзПГН. С уровнями АД в обеих группах наиболее четко коррелировали морфологические признаки, отражающие повреждения микроциркуляторного русла почек. ЗАКЛЮЧЕНИЕ. Полученные данные подтверждают то, что гипертонический нефроангиосклероз нередко сопровождается протеинурией, а тяжесть морфологических изменений в почках при АГ разной природы связана с уровнем АД.</p></abstract><trans-abstract xml:lang="en"><p>THE AIM of the investigation was to comparatively describe renal lesions in patients with arterial hypertension (AH) and mild proteinuria who had hypertonic nephroangiosclerosis or mesangial-proliferative glomerulonephritis diagnosed when investigating the kidney bioptates. PATIENTS AND METHODS. Under study there were 81 patients who had AH in combination with mild proteinuria that was considered as an indication to nephrobiopsy. The patients were divided into two groups of comparison: patients with essential hypertension (EH, n=42) in whom the morphological data had undoubtedly shown nephroangiosclerosis and patients with mesangial-proliferative glomerulonephritis (MPGN, n=39). Nephrobioptates were investigated at the level of standard light microscopy. Additional semi-quantitative assessment of some signs of lesions to the kidneys was made. RESULTS. Patients with EH of older age, had longer AH and higher values of arterial pressure (AP). Patients with EH had considerably lower glomerular filtration rate than patients with MPGN with comparable values of proteinuria. The degree of nonspecific glomerular, vascular and tubulointerstitial lesions of the kidneys in patients with EH was also higher than in patients with MPGN. More distinct direct correlation was noted between the levels of AP and morphological signs demonstrating lesions of the microcirculatory bed of the kidneys. CONCLUSION.
