Сравнить вазомоторную активность кровеносных сосудов у больных с атеросклерозом и хронической болезнью почек (ХБП) с гиперлипидемией, используя электрофоретические пробы с 0,2% раствором ацетилхоли на хлорида (эндотелийзависимая вазодилатация) и 1% раствором нитроглицерина (эндотелийнезависимая вазодилатация). ПАЦИЕНТЫ И МЕТОДЫ. Пробы с вазоактивными веществами проведены 20 пациентам с облитерирую щим атеросклерозом нижних конечностей (ОАСНК) II ст. по Fontaine (средний возраст 56±6 лет), 22 пациентам с ХБП (средний возраст 45±3 года) и 20 здоровым лицам (средний возраст 44±3 года). Микроциркуляцию в коже оценивали методом высокочастотной ультразвуковой доплерографии («МинимаксдоплерК», датчик с частотой излучения 25 МГц, лоцирующим ткани на глубину 5 мм). РЕЗУЛЬТАТЫ. У больных ОАСНК выявлено нарушение эндотелийзависимой вазоди латации, а у больных ХБП как эндотелийзависимой, так и эндотелийнезависимой вазодилатации. Изменения реактивности характеризуют сосуды микроциркуляторного русла, не пораженные атеросклеротическим процессом. ЗАКЛЮЧЕНИЕ. ОАСНК и ХБП сопровождаются системной эндотелиальной дисфункцией. Проявлением дисфункции эндотелия является измененная реактивность сосудов, в том числе на уровне микроциркуляторного русла.</p></abstract><trans-abstract xml:lang="en"><p>THE AIM of the investigation was to compare vasomotor activity of blood vessels in patients with atherosclerosis and chronic kidney disease (CKD) with hyperlipidemia using electrophoretic tests with 0.2% solution of acetylcholine chloride (endotheliumdependent vasodilatation) and 1% solution of nitroglycerin (endotheliumindependent vasodilatation). PATIENTS AND METHODS. Tests with vasoactive substances were made in 20 patients with II stage obliterating atherosclerosis of lower extremities (OASLE) by Fontaine (mean age 56±6 years), 22 patients with CKD (mean age 45±3 years) and 20 healthy subjects (mean age 44±3 years). Microcirculation in the skin was assessed by the method of high frequency ultrasound dopplerography («MinimaxdopplerK», transducer with radiation frequency 25 MHz monitoring the tissue 5 mm deep). RESULTS. Patients with OASLE had impaired endothelium dependent vasodilatation, and patients with CKD had impaired both endotheliumdependent and endotheliumindependent vasodilatation. Changed reactivity was characteristic of microcirculation bed vessels not involved into the atherosclerotic process. CONCLUSION. OASLE and CKD are accompanied by systemic endothelial dysfunction.
