Изучить степень корреляции количественных и функциональных показателей активности нейтрофильных гранулоцитов с функциональным состоянием почек и биохимическим составом крови при мембранозно-пролиферативном гломерулонефрите 1 типа(МПГН 1 т) у человека. ПАЦИЕНТЫ И МЕТОДЫ. Исследование проведено на 20 больных МПГН 1 т. Содержание катионных белков нейтрофилов изучали с помощью лизосомально-катионного теста (ЛКТ). Экспрессию эритроцитарных, комплементарных и Fcy-рецепторов на мембране нейтрофилов исследовали методом розеткообразования. РЕЗУЛЬТАТЫ. Величина цитохимического показателя ЛКТ связана положительной корреляцией с уровнем лейкоцитурии и отрицательной - со скоростью клубочковой фильтрации (СКФ) и суточной протеинурией. Интенсивность экспрессии Е-рецептора на мембранах НГ коррелятивно связана с концентрацией сывороточного креатинина. Активность экспрессии СЗЬ-рецептора на мембране НГ коррелятивно связана с изменениями СКФ. Функциональные изменения НГ коррелируют с СКФ, интенсивностью лейкоцитурии и концентрацией сывороточного креатинина, зафиксированных спустя 0,5-2 мес. после исследования НГ По-видимому, изменения СКФ, уровня лейкоцитурии и концентрации сывороточного креатинина были отсрочены во времени, исследовали изменения активности НГ периферической крови, а не инфильтрата почечной ткани, в которой развивается патологическая реакция. ЗАКЛЮЧЕНИЕ. Представленные в работе данные дают возможность предположить участие нейтрофильных гранулоцитов в механизмах повреждения почечной ткани при МПГН 1 т. Обнаружение сочетанных изменений активности экспрессии Е-рецепторов наНГ и величин цитохимического показателя/I KT может служить прогностическим признаком при данной форме заболевания, свидетельствуя о возможном последующем ухудшении функции почек.</p></abstract><trans-abstract xml:lang="en"><p>THE AIM the investigation was to study the degree of correlation of quantitative and functional indices of neutrophil granulocytes (N lG) with the functional state of kidneys and biochemical composition of blood in I type membranous-proliferative glomerulonephritis (MPGN) in man. PATIENTS AND METHODS. The investigation was performed in 201 type MPGN patients. The content of cationic protein neutrophils was studied by the lysosomal-cationic test (LCT). Expression of erythrocyte, complement and Fcy-receptors on the neutrophil membrane was studied by the rosette-formation method. RESULTS. The value of the cytochemical LCT index positively correlates with the level of Ieukocyturia and negatively - with the glomerular filtration rate (GFR) and circadian proteinuria. Intensity of the E-receptor expression on NG membranes has a correlative association with the concentration of serum creatinine. C3b-receptor expression activity on the NG membrane is correlatively associated with the changes in GFR. The functional changes in NG correlate with GFR, intensity of Ieukocyturia and concentration of serum creatinine revealed 0.5-2 months after the investigation of NG. The changes of GFR, levels of Ieukocyturia and concentration of serum creatinine appeared to be delayed in time, since the changes of NG activity in peripheral blood were investigated ratherthan renal tissue infiltrate where the pathological reaction developed. CONCLUSION. The data presented allow a suggestion of the participation of NG in the mechanisms of lesions of the renai tissue in patients with I type MPGN.
