ЭПИДЕМИОЛОГИЯ ХРОНИЧЕСКОЙ БОЛЕЗНИ ПОЧЕК У БОЛЬНЫХ С ХРОНИЧЕСКОЙ СЕРДЕЧНОЙ НЕДОСТАТОЧНОСТЬЮ

Аннотация

Имеющиеся данные свидетельствуют, что распространенность хронической болезни почек (ХБП) среди больных с хронической сердечной недостаточностью (ХСН) составляет от 9,2 при ХСН, обусловленной врожденными пороками сердца, до 71,2% – при ХСН, обусловленной ишемической болезнью сердца (ИБС) и артериальной гипертонией. Снижение функции почек ассоциировано с возрастом и функциональным классом ХСН. Однако, если низкую частоту ХБП среди больных с ХСН с врожденными пороками сердца можно объяснить молодым возрастом и нетяжелой ХСН, то причины значительного различия в распространенности ХБП у больных с ХСН, обусловленной ИБС и артериальной гипертонией, остаются неясными. Требует уточнения распространенность ХБП как среди больных с ХСН в целом, так и в отдельных группах больных. Необходимо уточнение причины гендерных особенностей ХСН, ассоциированной с ХБП. Целесообразно проведение не только клинических, но и клинико-экономических исследований, при этом следует учитывать, что результаты, полученные зарубежными исследователями, нельзя безоговорочно использовать в России, так как имеются значительные отличия как в организации медицинской помощи населению, так и в спектре медикаментозных препаратов, особенно препаратов-дженериков, используемых при лечении ХСН.
