ОСТРОЕ ПОВРЕЖДЕНИЕ ПОЧЕК И БЕРЕМЕННОСТЬ

Аннотация

Острое повреждение почек (ОПП) в акушерстве – серьезное осложнение беременности, родов и послеродового периода, которое может оказывать негативное влияние на исход беременности как для матери, так и для плода. В настоящее время в развивающихся странах частота ОПП во время беременности и в послеродовом периоде составляет 4–26% всех беременностей, а в экономических развитых регионах мира – не более 2,8%. Частота акушерского ОПП, требующего проведения диализа, равна 1:10 000 беременностей. Причинами повреждения почек у беременных могут быть гипертензивные расстройства, различные варианты тромботической микроангиопатии (HELLP-синдром, атипичный гемолитико-уремический синдром, тромботическая тромбоцитопеническая пурпура, катастрофический антифосфолипидный синдром), острый жировой гепатоз беременных, акушерские кровотечения, сепсис и септический шок. В статье рассматриваются критерии диагностики, биомаркеры, сроки развития ОПП при беременности, принципы лечения и профилактики данного осложнения в акушерстве.
